Consistently, the best I have is from Freddy's

In my life, though, about ~3 years ago or so, I went to Steak and Shake of all places, and had the most divine and best burger I've ever had in my life. I've been to S&S before and since and they're normally pretty good, especially for the price, but there was just something about or in that burger that day.

Burgerville in Albany, Oregon does a fine example:
great beef, Tillamook cheddar, pepper bacon....for an everyday $5 burger it's in for honorable mention;

Fast food burger is definitely Culver’s. Will have to try Freddie’s.

A few months before I left my previous airline, we had an overnight in Gulfport, MS...hotel was on Beach Blvd. A short walk past Hard Rock Casino, we hooked a left up Main St. and went to Mugshots Grill & Bar...
BEST.
BURGER.
EVER.

The secret was the bun...perfect texture, taste, and "chew". Asked the waitress and she said they made the buns in-house and they were "sweet sourdough". Didn't know there was such a thing. Anyway...it left enough of an impression for me to talk about it over 2 years later...

Oklahoma Onion Burger, in El Reno Ok.

The Oklahoma Onion Burger was originated during the Great Depression years.

Very thinly sliced onions, paper thin sliced is best, are smashed into the meat on a very hot grill.

If you've never had one then I can't really describe it.

The Arc burger in Costa Mesa, CA. Nothing is frozen and everything is cooked over wood.

$25 and I'd gladly pay more if they ask.

Best would be, like a couple others have said, the ones I make myself on my Weber kettle. But for go-out-and-eat-it burgers, there's a place on the other side of town called The Hangar. No it's not on an airport but they do have some models hanging from the ceiling. They have one the name of which I've forgotten, but it has almost literally everything one it. Up to and including a fried egg. Just make sure you're hungry if you order it.

[ETA: "Babe's Burger", that's the name of it. Named in honor of the Sultan of Swat himself.]

The other place I keep going back to is called Killer Burger. It's a chain, but a local one - Portland and Eugene. They've got all kinds of weird combinations, but one or two of the more "normal" ones are damn good. The store I started going to is downtown, walking distance from my office, but currently closed according to their web site, I assume because of its proximity to the rioting mobs. But they opened two new ones out west closer to my place, I need to get over there one of these days soon.

The black and blue bacon burger at Rojo in Birmingham comes to mind for consistent excellency. I'm a sucker for blue cheese and bacon on a burger. I also had a burger at Bobby Flay's place in Vegas that was sublime, but that was a one time thing.

The bacon cheddar burger at a restaurant called Main Street USA in Ocean Township New Jersey.
The cheddar is a cheese sauce the make right there in the restaurant. We would ask for extra cheddar sauce on the side for our french fries.
Its been 8 years since I had one.

Personally, I think cheese has no place on a burger. Fresh, lean meat grilled to perfection on a bun, with some lettuce and tomato, with a dab of mustard does it for me.

That said, last night I made "'Shroom 'n' Swiss Pork Burgers," a recipe I purchased from the Hello Fresh meal subscription service. They were outstanding! You could have bowled me over with a feather: PORK instead of beef? Yep. And with a grilled mushrooms, swiss cheese and a sour cream-honey mustard sauce. Delicious!

Probably Johnnys Grill, they make a really good Oklahoma Onion Burger.

Right now Freddy's is serving their Green Chile Cheeseburger. It's a seasonal thing, as long as they can get fresh Chiles.

I had one yesterday. I was really good. I think I'll splurge the calories and get another tomorrow.

If you have a Freddy's near you and they have the Green Chile Cheeseburger, try it.

There seem to be a ton of sit-down full service specialty burger places, and they're all really competing hard for business right now. As far as fast food, I rarely eat it, but my wife and I stopped at a Culver's on a trip a couple of weeks ago for the first time, and had their Butter Burgers. Had to be the best fast food burger I've had in a really long time - maybe forever. Better than the Shake Shack and Five Guys in my area
