Never read the book (may fix that eventually), but as I was watching this on netflix - why couldn't she turn / twist and kick out one of the bed posts? Or, attempt to at least?

Wouldn't a dislocated shoulder be better than what eventually happened? Even getting movement to the outside, and couple that with pulling it back - maybe the nails would loosen.
