| I can't imagine what goes through the teams minds when this happens. "Did I do that?" comes to mind, an 80s nonsensical TV show line, but it has to weigh heavily on everyone. I am amazed that the space industry can sustain all these losses. If we were to calculate the probability of another failure, using the Poisson distribution, for low frequency defects, we would likely be horrified. If I were an astronaut, I would likely resign my commission, with this kind of track record.
