NTI Co-Chairman and CEO Ernest J. Moniz and Munich Security Conference (MSC) Chairman Ambassador Wolfgang Ischinger convened 19 current and former global leaders and experts for a March 17, 2021 senior leaders tabletop exercise focused on reducing high-consequence biological threats with potentially catastrophic consequences.

This third annual tabletop exercise organized by NTI’s Global Biological Policy and Programs team (NTI | bio) in conjunction with the MSC is part of the MSC’s “Beyond Westlessness: The Road to Munich 2021” campaign. This effort includes several virtual high-level events and initiatives aimed at advancing the security policy dialogue on priorities for a new transatlantic agenda and laying the groundwork for in-person debates in Munich later in the year. This year’s exercise was conducted on a virtual platform due to the COVID-19 pandemic.

The impact of COVID-19 provided a pressing backdrop for this exercise, as the ongoing pandemic has highlighted weaknesses in the international architecture for preventing, detecting, and responding to pandemic threats. This is an urgent concern because future pandemics could match or exceed COVID-19’s devastating impact in lost lives and shattered economies. Even more concerning is that there are critical gaps in biotechnology oversight that create opportunities for accidental or deliberate misuse with potentially catastrophic global consequences. This was illustrated in the exercise scenario: a localized bioweapons attack with a genetically engineered monkeypox virus begins in the fictional country of Brinia. Over 18 months, the scenario evolves into a globally catastrophic pandemic, leaving 40% of the world’s population infected and over a quarter billion people dead.

The fictional exercise scenario unfolded gradually through a series of short videos that participants reacted to during a facilitated discussion. Key themes emerged regarding the need to strengthen international pandemic risk assessment and early warning systems; to establish clear triggers for national-level anticipatory response and aggressive early action to slow disease transmission and save lives; to reduce biotechnology risks and enhance oversight of life sciences research; and to promote new and stronger international health security preparedness financing mechanisms.

A full report will be published later in 2021. More information about previous exercises can be found in final reports from 2019 and 2020.

Belgium introduces 21-day quarantine for all monkeypox cases

Belgium has become the first country to introduce a mandatory quarantine for people with monkeypox.

Those with symptoms will need to isolate until their sores subside, which is expected to be around three weeks.

Belgium’s Risk Assessment Group (RAG) and health authorities said those infected will need to isolate for 21 days.

They said anyone with symptoms should seek medical advice.

There has been an outbreak in the country linked to a gay fetish festival called Darklands, where three attendees tested positive.

Organisers said: ‘There’s reason to assume that the virus has been brought in by visitors from abroad to the festival after recent cases in other countries.’

They circulated warning information from the government saying that those infected would have to self isolate for three weeks and avoid any physical sexual contact.

Over the last week, concerns have grown about the spread of monkeypox, a viral disease which is usually rare and found mainly in those who recently travelled to Africa.

It has now been found in ten countries outside of Central and West Africa, including the UK where there have been 20 confirmed cases.

Dr Susan Hopkins, a chief medical adviser for UKHSA, said updated figures for the weekend will be released on Monday as she warned of more cases ‘on a daily basis’.

Monkeypox has an incubation period of five to 21 days, so people have been warned to be vigilant for three weeks after they ay have been exposed.

Dr Hopkins said: ‘The community transmission is largely centred in urban areas and we are predominantly seeing it in individuals who self-identify as gay or bisexual, or other men who have sex with men.’

Asked why it is being found in that demographic, she said: ‘That’s because of the frequent close contacts they may have.

‘We would recommend to anyone who’s having changes in sex partners regularly, or having close contact with individuals that they don’t know, to come forward if they develop a rash.’

The World Health Organisation says that people can catch the disease from close contact with skin lesions of infected people, from inhaling respiratory droplets or touching a contaminated object (such as a towel or bedding).

They said that transmission via droplet respiratory particles ‘usually requires prolonged face-to-face contact, which puts health workers, household members and other close contacts of active cases at greater risk’.

The United Nations’ Aids agency (UNAIDS) on Monday denounced some reporting on the monkeypox virus as racist and homophobic, warning irresponsible language was undermining the response to the outbreak as it gathers pace across the globe.

UNAIDS said “a significant proportion” of recent monkeypox cases have been identified among “gay, bisexual and other men who have sex with men” leading to some portrayals of Africans and LGBTI people gravely “reinforce homophobic and racist stereotypes and exacerbate stigma.”

The UNAIDS deputy executive director, Matthew Kavanagh, advised such talk should cease forthwith. He said:

Stigma and blame undermine trust and capacity to respond effectively during outbreaks like this one.

Experience shows that stigmatising rhetoric can quickly disable evidence-based response by stoking cycles of fear, driving people away from health services, impeding efforts to identify cases and encouraging ineffective, punitive measures.

The agency further urged all media outlets covering Monkeypox to follow updates provided by the World Health Organization (W.H.O) while acknowledging the situation is “evolving and the surveillance expanding, it is expected that more monkeypox cases will be identified.”

The Biden administration has ordered an additional 500,000 more doses of the Jynneos vaccine for monkeypox, marking a big step up in the government’s response amid rising cases in the United States and around the world.

Denmark-based biotech group Bavarian Nordic, the manufacturer of the vaccines, said that the U.S. Biomedical Advanced Research and Development Authority (BARDA) has placed the order, to be delivered later this year.

“With the previous order from BARDA for 1.4 million doses of liquid-frozen JYNNEOS, awarded in 2020, this order will bring the total U.S. inventory of the vaccine to nearly 2 million doses,” the company announced on Friday. Many of the 1.9 million doses are being held by the company until the U.S. government requests them.

An active-duty service member based in Germany has the first known case of monkeypox, the US military has announced.

“We can confirm that a duty service member from the Stuttgart military community recently tested positive for monkeypox,” Navy Capt. William Speaks, a spokesperson for the U.S. European Command, said in a statement Friday, NBC News reported.

The soldier, who was not identified, was treated at the Stuttgart Army Health clinic and is currently in isolation in their quarters on-base, officials said.

“Public Health Officials have determined that the risk to the overall population is very low,” Speaks added. “As a precautionary measure, contact tracing is being done for clinic staff who interacted with the patient. The case in Stuttgart is of the West African strain, which is generally mild and human-to-human transmission is limited.”
