Not really worried about cost. Ideally, something which mimics a 45, but I’d like it as reliable as possible. I suppose it doesn’t have to be integral.

1) Determine a host pistol first. Unsure of "mimics" a 45. Do you mean like a 1911? Reliable 22's are Ruger MK's or Browning Buckmark. 2) Select a rimfire suppressor. Rimfire is dirty-lot of carbon and lead build up. Lightweight is best. Materials are titanium or stainless steel or aluminum. 3) NFA item. Be prepared for a long weight. This is the entry item into the NFA world. 4) Cost is not an issue--> Volquartsen Mamba or Scorpion, TandemKross Kracken.

For a number of reasons, I highly discourage you from pursuing an integrally suppressed .22 pistol. However, based upon your other requirements I recommend the Ruger 22/45 Tactical model. This variation is factory threaded, has a grip angle which mimics the iconic 1911 and is superbly reliable out of the box. For added shits & grins it can be upgraded with a host of premium Volquarten parts and accessories. Two of the finest rimfire silencers available are the Dead Air Mask and the Silencerco Sparrow. Both are top tier options, though the Mask does a better job of eliminating the annoying "first round pop" often associated with pistol length barrels. Attached is a photo of a Ruger MK IV Tactical coupled with a Silencerco Sparrow. This particular pistol has been upgraded with Volquartsen's Pistol Competition Kit, Tandemkross' red anodized trigger & thumb safety and a pair of VZ double-diamond G10 grip panels.

You need to look up TBA suppressors, and in particular their 22/45 Sicario offering. I think this exactly meets your needs. One piece core in stainless making clean up a snap in dirty 22 LR. TR

Wow! This is certainly game changer in the world of integral .22 cans.

I’ve got several 22 pistols I shoot with a SIG SRD 22X suppressor… the best and most reliable, by far, is my CZ Kadet. I bought the Kadet Adapter and then sent it to Tornado Technologies to have them add the threaded end to the barrel. The beauty is that it fits any of the CZ-75 and 85 models. I’ve used it on my PCR, P-01 and SP-01 with equally good results. I haven't tried it on my Rami or Shadow pistols, but it should work on them as well. I’ve used a bunch of different ammo brands without an issue. Here it is on my SP-01: We the people are the rightful masters of both Congress and the courts, not to overthrow the Constitution but to overthrow the men who pervert the Constitution. I personally run a DA mask on lots of .22lr pistol stuff, but I see the attraction of an integrally suppressed option. What exactly would you consider the issues? They seem to cut the length etc. and I can't see the downside. Example to use like the TACSOL one for the 22/45. I've lots of experience on .22 suppressors and I see nothing particularly problematic in looking at this other than price which the OP says is not an issue. “So in war, the way is to avoid what is strong, and strike at what is weak.”

I was thinking of the old days when integrally suppressed .22's were difficult to maintain and service. Apparently, silencer technology has advanced to address these concerns. While I personally prefer multi-host capability, I can appreciate why some shooters are attracted to an integrally suppressed firearm, especially one which is user serviceable.
