I've had x2 instances.

One I was shooting factory loads in my G27 at a Forum event at a Marzy / Cockatoo BBQ & Bullets Event. Can't remember the make. But they were JHP factory loads of some sort.
One was over pressured. Blew the extractor into orbit and launched the mag out of the gun. "Welded" the brass in the barrel chamber. Blew out the back of the case. That gun was absolutely %100 out of the fight. Stung the hell out of my hand and reminded me why I wear shooting glasses.

2nd I had with my AR. I had some crappy Independence 55gn rounds. Was popping primers. Primer got caught in the barrel lugs of the AR. Bolt wouldn't go into battery. Took me a 3-4 minutes to find the primer and clear it. Would have been long enough in a fight to get killed plenty times over.

Train how you intend to Fight

Remember - Training is not sparring. Sparring is not fighting. Fighting is not combat.
