Based on what I know about the event, there were plenty of indicators, but the ultimate decision-makers, including in the intelligence community, misinterpreted or chose to ignore them. That has been the story about many so-called intelligence “failures” easily going back into the early 20th century. The ones that readily come to mind are Germany’s attack on the Soviet Union in 1941 and a few months later at Pearl Harbor*. Evaluations and analyses are almost always influenced by preconceptions—if not simple wishful-thinking—about intent and capabilities.
Gathering intelligence is usually the hard part, but not always, and the mistakes are usually made in interpreting what was gathered.
* I have read many accounts about the intelligence available prior to the attack on Pearl Harbor, and the thinking and decisions made by the US leadership on its basis and the basis of mistaken ideas about Japan’s capabilities. The one I’m reading now, though, is probably the best: Sealing Their Fate: The Twenty-two Days That Decided World War II by David Downing.

Interesting site link below for reporting on the Iran situation. Apologies if this has been referenced/posted prior.

I wonder too. Hopefully they'll be taken out soon, but if not, maybe they'll see the futility of their efforts and turn tail.

One YouTuber has stated that for years Iran has been emphasizing the philosophy and practice of allowing lower-level military officers to decide and act on their own.
I was a little surprised to read that, but after some thought I realized it’s nothing new in the wars civilized nations have been fighting against a bunch of murderous fanatics for years. Those men know that they are carrying out their deity’s will, but they also realize that he won’t necessarily step in to do what he expects them to do; i.e., like so many divinely-ordered actions, it’s a test to see if they deserve the rewards that (could) await them.
In the case of Iran, I suspect they realize that part of their deity’s test is the possibility of what has happened over the past few days because it’s happened on one scale or another countless times in the past: “Don’t rely on your leaders being available to guide you on your holy mission; we may not be here and will be looking down from Paradise on you.” That’s the whole basis of the so-called “lone wolf” idea.
These people are misguided fanatics, but there’s no reason to believe they’re stupid.
Oh, yeah: How about that practice of supporting (or not supporting) the wrong guy? The US can point to plenty of its own examples, but the one example I recall most often was that the Germans allowed Lenin to cross Germany to Russia from Switzerland in WW I with hopes he would hamper the Russian war effort. In that at least they were more than a little successful.

All indications is anything floating from the Iranian Navy of any size was hit or, sunk, however we also know the Iranian Navy isn't the thorny problem in the strait as its the fleet of Boghammers and other fast-boats of the IRGC Navy that are the concern. Nothing has come up about the Behshad...yet. However Iran's converted oil-tanker into a drone aircraft carrier (the thing looks like a 8th graders idea) was hit and we've yet to see any video of imagery of that strike.
This whole thing is starting to remind me of that scene in the movie Enemy At The Gates:
Officer #1: [repeating through megaphone] The one with the rifle shoots! Officer #2: [handing out rifles] One out of two gets rifle. Officer #1: The one without, follows him! When the one with the rifle gets killed, the one who is following picks up the rifle and shoots!
At least you didn't use the identical link quoted in V-Tails post (this time!). It definitely seems a whole lot less adolescent now that you deleted your original post so as to gain at least one other post as a buffer from the previously linked 'rick-roll'.
