My Mom's beef stroganoff with fluffy white rice, fresh crescent rolls, and white chocolate/raspberry heaven cake, which I've had for most birthdays since 1992. But my Mom passed in 2019 so...not happening. Barring that, maybe beef fajitas from Uncle Julio's (which my wife and I just had for our wedding anniversary last weekend).

I'd start with a bowl of lobster bisque then a lobster and scallop po boy and my wifes potato salad. For a veggie, I'd have green beans with bacon and for dessert I'd have huckleberry cobbler with vanilla ice cream.

My mom would make that for my birthday each year I was home.
For those not in the know, "country-fried" steak is thin round steak that has been rolled in flour, which is then beaten into it with a special mallet (or the edge of a heavy saucer), and then fried in a cast-iron skillet until crusty and brown. The leavings in the skillet are mixed with milk and flour to make the gravy. (My arteries are hardening even as I type this.) That steak is good hot or cold, eaten from a plate or in a sandwich, or just hand-held. Yumm!

This may be something that will change over time for me. But today I would want one of these two courses (it's been consistent for the past 10 years or so when this question is posed; Italian and Japanese are my favorite cuisines; and as much as I like steak and BBQ, I think I would prefer seafood for my last meal):
1) appetizer: hon-maguro o-toro sashimi (bluefin tuna belly sashimi); soup: asari sakamushi (clams simmered in sake based broth); entree: mentaiko onigiri made with toyama rice and katsuo tataki (bonito seared with straw); dessert: shaved ice with fresh japanese mango.
2) appetizer: fritto misto calamari; soup: clam chowder; entree: cioppino (heavy on shelled crab, clams, mussels, prawns, light on fish) with a freshly made, soft rosemary baguette; dessert: souffle with a dark choco hazelnut drizzle.
