From the pictures, did the Sig fail to feed the second round?

Just a wag, but if he jammed the Sig hard up under his chin and fired, the Sig probably could not cycle properly if it did one of the things you suggested. Again, just a guess
Could also be due to limp wristing from a weakened grip from being injured/slippery with blood.
I’d buy the limp wristing preventing proper cycling, but not sure I’d expect a contact shot to affect cycling unless he held it in battery for the shot. With a contact shot, the slide can be pushed back out of battery, preventing the weapon from firing. Some folks teach holding the slide forward in battery with the support hand to ensure the contact shot works. This does prevent cycling though. It might be kinda tricky to hold a hammer fired P226 in battery with the thumb of a bloody hand, but he may have done 5hat.
