Let me preface this with I am not a doctor and you should fully consult your attending physician or orthopedic surgeon for any questions regarding this. This is a very basic translation of the notes only, and based on common scenarios. You results may vary.
1.1 x 1.0 cm (AP x ML) full-thickness tear of the anterior supraspinatus tendon at the footprint. A hole in the tendon on the front side appx 1cm around.
Moderate infraspinatus tendinopathy, and interstitial partial tearing of the musculotendinous junction involving 50% thickness. 2.1 x 0.4 Moderate wear with a 50% thickness tear that isn't a complete hole
cm intramuscular ganglion cyst in the deep infraspinatus muscle fibers.Fluid pocket in the muscle caused by wear/tear or stress to the muscle
Treatment usually starts with ice/head, PT, possibly steroid injections.
If that doesn't work after a couple months or depending on the MD's other findings, surgery may be warranted.

lIf it hurts all the time or prevents you from living normally, probably yes. Shoulder surgery is so advanced these days I wouldn't worry about it unless what you described is really bad in the world of shoulder problems and is considered inoperable.
Sometimes you just need joint surgery and it is what it is. My wife has a girlfriend who needs a hip replacement, but she doesn't want surgery. Downside is she's basically crippled, in constant pain, and always hindered in daily life. I don't get it myself but I know lots of people are like that, for even worse things, though I've never met anyone who had a hip replacement that wasn't somewhere between happy and overjoyed with the result compared to their previous state.

I'm seeing an Ortho doctor later this week. This occurred when my Volvo got hit, end of August. Fortunately my hip replacement was not damaged, just muscle ache.

You completely tore a RTC tendon where it attaches. You almost tore a 2nd RTC tendon at the area where the muscle and tendon meet. The cyst has probably been there as a result of earlier shoulder trauma. As for surgery, depending on if it's or dominate shoulder, what you do, your age, etc.....but if you are active and it's your dominate shoulder most likely, Yes.

I don't have the notes in front of me but my wife was in an auto accident in June . The Ortho Dr. said it was a " partial tear in her Rotator Cuff ". Injections did nothing and she finally had surgery three weeks ago .

I tore my rotator cuff in June 2023. Hurt like hell and PT really, really helped. Doc said while the PT helps, the only real "fix" is surgery.
I had surgery April 2024 and did PT for 5 months afterward. It's much better, but I'm still careful with it.
My suggestion would be do as much PT as your insurance will allow for. It was well worth the price of admission, IMO.

The supraspinatus tendon is one of the 4 tendons that compose the rotator cuff (RC). Its action is to move your arm out to the side, called ABDUCTION. With a full-thickness tear, you will barely be able to perform this action, if at all, during the initial stage of the injury. There have been folks with this type of injury who healed completely without surgery, with just conservative treatments (meds, PT, injections), but those are the minority.
The infraspinatus tendon is the 2nd tendon of the RC. Its action is to help you rotate your arm externally (OUT). Looks like you have a 50% thickness tear at the junction of the tendon and muscle. The ganglion cyst usually is an incidental finding that is unrelated to the injury. With more than one tear that are significant as in your case, eventual surgery is likely needed. But, your orthopedic doctor will explain the treatment plan in details.
The other two tendons are the teres minor, a minor tendon that also helps externally rotate your arm, and the subscapularis which helps rotate your arm internally (IN).

I went to the range this morning, Full of HOPE...turned out it was NOPE. Pain getting into proper form. Fired 10 rounds Off hand (left) unsupported and came home. Yard work has been on hold since the injury.

I hope you have a good doctor. I tore my rotator cuff four different times and every time I had an MRI and was told it would heal. After many years, I began to develop horrible pain in my upper back near that shoulder. My gut said my rotator cuff didn’t heal and my other muscles were compensating. I finally saw one of the best shoulder doctors in my city and when he looked at the MRIs, he said I should have had surgery from day one and now after so long he didn’t know if he could fix it. He did the surgery and I was back to normal and the back pain was indeed muscles compensating.

Yeah after surgery, it was a full year until I was 100% back to normal with that arm and it was my dominant arm.

I had rotator cuff surgery on both shoulders in college. It's not perfect but it works. But these days the methods used are much better and I would not hesitate one moment to fix a bum shoulder.
I think they take a while to heal because those joints are so mobile and heavily used compared to other joints in the body.
Mine both took a while to heal back then and I was young.

I'm no physician, but five or six friends over the past few years who have had this type of surgery have ended up with around 10 days for the worst of the psin, then VERY slowly getting better.
