Negligent discharge today
I have never had a ND, but I have had a couple ULG incidents.

"Unexpectedly Loaded Gun"

You pick up a gun that you know is unloaded, but from force of habit drilled into you, you clear the "unloaded" gun, and that's when a round pops out.

Not a ND, but still a pucker moment for me and a reminder that "All guns are always loaded, even the unloaded ones." (Maybe, especially, the unloaded ones.)

One of my practices is to take the ejected round and put it on top of the previously ejected magazine. The idea being that if you don't see a magazine on which to set the ejected round, maybe it's still in the gun along with a fresh round in the chamber.

That's it right there, IMHO. Lack of focus (mind wandering), distraction, and deviating from habit, all leading to a surprise.

Thanks for sharing. We are all human and mistakes happen, luckily this one was pretty benign besides ringing ears and patching a hole.
