As a retired anesthetist, I can tell you that I often asked the same questions twice.....you would be surprised how many times I got different answers ! Would you prefer that I just accept any answers that I find on the almighty, all-knowing computer ?
Is it clinicians entering the wrong info or patients giving incorrect answers ?? I don't know.....I do wish that I had a nickel for every time a patient has told me that he had no history of heart problems, only to find a scar in the middle of his chest from a coronary bypass once he was asleep..

Since 7am I have become addicted to alcohol, started using illicit drugs and am now smoking 2 packs of cigarettes a day. Oh yeah, I forgot, I now have an allergy to latex. Other than that nothing has changed in the last 3 hours.

StorminNorminI absolutely no longer fill out the online paperwork for a doctor appointment. Every time I have done it, when I get to the office they hand me a stack of paperwork fill out all of which is exactly what I did online to save time. They tell me they don’t get that and need the paperwork filled out. Where the hell does the online paperwork go then? They don’t know. So no more and I just get there early to fill it out. Last time at a new doctor they gave me nothing to fill out and I was surprised. Then when I was READY TO LEAVE at checkout, they ask me if I turned in my paperwork. I told them they did not give me any paperwork and you are the person who checked me in. They then handed me a stack of new patient paperwork that I had to sit there and fill out despite arriving early to do it.
Service is horrible everywhere today. Now I appreciate why before going into surgery or something serious, every person you encounter asks you your name, DOB, and exactly what procedure you are having and where.

TRIOMy last visit, I arrived too close to the scheduled time. No time to fill form, so I folded it into my pocket. After speaking with three people, no one asked for it. I seriously think they just give this to us to pass the time as we wait for our appointment.
In 35 years, only ONE Dr looked at that form and asked me questions of my replies.

She asks questions because it’s her job. Other people she deals with may not be as brilliant as you, some people present themselves and forget what meds they were on. Didn’t write down they have a condition “that’s why I’m here”
