mr kablammoA claim denial rate almost 1/3 ?!?!?
Holy fucking shit!! More like theft than insurance! Killing the dude may be a tad extreme but reorganization or shuttering of the company seems reasonable, even compelling.

PASigHealth insurance companies have become the Robber Barons of the 21st century and I think there isn't an industry more despised right now.

architectOne detail I haven't seen discussed. Who was filming the incident in the original Twitter-posted video (since deleted, but probably available somewhere)? It appears to have been made to appear as if it were taken from a fixed-position security camera yet after the shooter starts to leave the scene, the camera pans to follow him, then stops abruptly as the cameraman realizes he shouldn't be doing that if he wants to maintain the illusion.
Or am I so out of the loop to not know that security cams will follow movement?

I have a G21 that performs without issue with a Rugged Obsidian, which is a very large can.
Because your Obsidian 45 has this piston/booster as part of its design:

it looked like someone just took a phone video of the screen showing the played back security footage.
I've personally seen a cop do this. held his phone near a screen and got the footage he wanted.

That is exactly what happened. And even if that wasn't, there is such a thing as PTZ cameras...

On my aged AAC EVO-9 there is both a piston and a booster spring for use If you’re shooting a fixed barrel (e.g., Beretta M9). It won’t reliably cycle without the spring.
As for it being a Welrod, those typically go for > $30,000, a VP9 is $9,000-$10,000, and even a B&T is at least $2,000. I don’t see an amateur putting out that kind of money for a pistol, nor do I see a professional (which this schmendrick ain’t or he’d have tested it first and spent time learning to shoot) using so unusual and traceable firearm.
I’m leaning toward an anti-capitalist, anti-private health insurance, anarchist type, a lá ANTIFA or Occupy, with a grudge against the company.
