Having lost a kidney, I have to avoid NSAIDs because they are processed more through kidneys and taxing what's left of my one. HBP can kill the kidneys quickly. Happened to an aunt of mine. She had no idea she had she had HBP until she started to get sick from kidney loss.

We talked to the nephrologist today and he is thinking it's Minimal Change Disease and has ordered a kidney biopsy ASAP.

I'm not a doctor but wait a sec. He presented first with hypertension? No severe peripheral edema? How were the blood tests - okay for most except albumin and BUN/creatinine? Just curious about MCD vs acute renal failure proximate to use of allergy meds and Advil while possibly dehydrated.

The biopsy is scheduled for Tuesday but we picked up his prescription early. We were told to not start the prednisone until after the biopsy.

I asked the nephrologist if he is to take 20mg three times a day and he said yes. However, the prescription bottle says to take all three pills after breakfast for 60mg all at once. Which is correct? I'll call the doctor on Monday but am just wondering which sounds right.

Depends on conditions and severity, but typically, outpatient prednisone therapy is once a day dosing. For example, for asthma flare-up requiring prednisone, it's 50 - 60mg once a day.

But, always clarify with your kidney doc, first. But, if I had to bet, it's 60mg once a day, because the pharmacist had to follow the exact direction as written on the nephrologist's prescription. They cannot just take the liberty of changing it, without first consulting with the doc.

60 is a pretty good dose. I seem to recall that generally a single dose in the morning is advocated for prednisone over multiple and/or evening doses.

If it really is MCD, you may want to consider other immunosuppressants and get off prednisone quickly. Be wary of diuretics as well as some may have a propensity for gout.

It seems it's the maximum daily dose. I think the doctor said it's a 6 week treatment with the prednisone.

Sounds about right. If it's MCD and signs of remission are evident in 2-3 weeks, the remaining weeks are likely just taper dosages to avoid things like Addison's.

Yeah, we transplanted quite a few who O/D'ed on Tylenol for various reasons. Want to really kill your liver? OD on Tylenol and have a little booze. Fortunately, some could be salvaged before transplant was required by some really sharp hepatologists.

Hope this guy does well and the biopsy shows what they think it will.

I'm getting concerned about my boy having this biopsy. It's scheduled for tomorrow morning.

Is this a generally safe procedure? I'd hate for it to make his issue worse. Am I just worrying unnecessarily?

I think it's generally safe; the biggest concern is cutting a blood vessel which results in a major bleed. So should be an overnight stay to monitor for internal bleeding.

Other than that, he may or may not end up with a sore back from the puncture through back muscles

There's no way he's going to stay idle for an overnight stay. The registration office told me it's a short stay procedure lasting 45 minutes. This hospital also has poor reviews. If we lived near another, better rated hospital, I'd not be so concerned.
