Suggestions for a useful order form Login/Join

Our company uses google for our email, etc. As such, we have access to google forms. I've been playing with them, but I have not been too happy.

I'd like a form where people indicate the following:

New Customer - (if new, I need them to submit a photo of their retail license/health code sticker/the retail license number, and delivery address. Aside from the delivery address, the rest can be manually dealt with)

Existing Customer
A list of products, where they can indicate the desired quantity.

Days of the week, and times, when they receive deliveries of ice cream

Contact person
Contact person's email/phone number
Billing Contact, if different.
Delivery instructions/comments, etc.

For some reason, the hardest seems to be finding a form where people can go down a list, indicating how much they would like of each product.

Are you looking for an existing form or how to make your own form? For every form I use, I custom make using MS Excel but even the "free" programs like LibreOffice can be used to "make-a-form".
