My take on horrible diseases throughout history is that they never completely disappear, they are still around in one form or another waiting to bust out again.

A lot of them were bacterial, and were conquered by antibiotics (think Black Plague). But bacteria are developing immunity to existing antibiotics, and the drug industry doesn't make enough money on antibiotics to make it worth their while to develop new ones. So look for some of the oldie and baddie diseases to make a comeback.
