I use the BAFX 34t5 Bluetooth on my Android phone with the TORQUE interface app. It has always worked flawlessly and will do all that you've asked for. I got mine from AMAZON.

I currently use a DashBoss on my company vehicles. It is a little more expensive than the cheap models but it works well and finds more codes than the cheap ones do particularly for a diesel engine. I don't go on any long trips without it.

I have a BlueDriver. It diagnoses and resets codes on my 2000 Chevrolet.
