Just what exactly is this, I don't recall ever seeing this option on things like drivers license, employment applications, medical forms, etc. I guess they leave that area blank.

When my two youngest were in their early teens they would go to the soda fountain and grab the largest cup. They would fill it with every flavor of drink in the fountain. They called it a suicide.
