I went looking around the net and there is a licensed version of the watch for sale out of the UK.
Granted it is battery operated and all, but still.
It also includes the watch chain and a picture of the sister as shown in the film.

I ordered one just because I can.

My guess is that there was never a music box watch as represented in the film.
They probably used a pocket watch with the woman's picture in it and overdubbed the sound of the watch using Ennio's work.

Jim - please provide us with more details on this. I'm not familiar with the symphony. But that doesn't look like an easy thing to pull off and I'm wondering how he could get away with doing that. I can't see the typical symphony attendee being interested in that.
