If you really care about the details, you would know that it was a classic case of someone who had quit, who starts using again, taking what they think they need to get high, but underestimating what their limbic system could handle.

This is how most traditional overdoses with opiates happen; it was until illicit fentanyl from China/Mexico made it into the USA in such huge quantities.

Other people around him had buprenorphine available for him to take instead, which would’ve saved his life likely.
