Read a study lately about businesses getting more benefit from changing default from Opt out to Opt in. People take the path of least resistance.

Received two boxes of CPAP supplies from Apria. Neither was wanted or needed. Apria just rolled all customers into auto replenishment. 30 minutes on the phone to correct and I have to call back again after the merchandise is received back at Apria to get the credit charges reversed.
Be aware that this is likely the way all consent forms are going to be handled
