Everything I see says 2 form 4s, 2 pictures, and 2 fingerprint cards.

The form 4 instructions say: duplicate form 4s, attach a photo "here" to this form, attach 2 fingerprint cards.

So, do I need two fingerprint cards (1 per form 4) or 4 (two per form 4)?

Silly I know, but am and getting the cards done by a friend/LEO and need to know how many.

Also for a C&R license (filling it out tonight - required for Missouri) are any other forms required? the 5300.38 seems to pertain to a business and 5300.36 and 37 seems to have disappeared.

I shouldn't need 5330.20 for the C&R, since it seems to have one built into the form.

Whoever writes the instructions for the ATF form really tries to confuse a person.

I always send two fingerprint cards per form 4 submission (ATF keeps one form 4 and sends the other back to you)... I've been told by a dealer that if you're purchasing multiple items at once then you can submit two fingerprint cards for everything. I went the safe route and submited two fingerprint cards for each pair of form 4's, check, & citizenship form... but I'm a luddite like that.
